Consultation Agreement

Below is a copy of the consultation agreement that will be sent before every initial consultation.  This is found within the terms and conditions when booking an appointment and must be signed before completing the booking. If failed to do so a copy will be sent to the client via email If this is not completed 24 hours before the session starts then the session will be cancelled and no refund will be given. After the initial acceptance, clients won't see the agreement but will be directed to this page to be reminded of what they have signed and agreed too. 

I understand that nutrition consultations are not a substitute for professional medical treatment and that the nutritionist does not diagnose medical conditions, but may help manage them through dietary recommendations.

 

These consultations are also not a substitute for professional counselling or therapy. I therefore understand that Scarlet may refuse to continue the consultations to me (the client) if she deems it inappropriate due to a lack of medical treatment for a serious condition, or any other reason.

 

I understand that if I have an active eating disorder my nutrition consultations may not be able to proceed until I have written permission from my doctor or therapist/psychologist.

 

I understand that Scarlet may make handwritten notes during the session, and that these details and conversations will be kept completely confidential and stored securely.

 

Scarlet retains a record of my name, contact details and dates and times of appointments on a password-protected database.

 

Scarlet is required to keep brief notes about the focus of the sessions. These are kept securely and are password protected.

 

I understand that if Scarlet deems me to be in immediate danger to myself or others this confidentiality may need to be broken.

 

I understand that payment must be made before each session or the session cannot take place.

 

I understand that I am responsible for my own Internet connection. Time lost during a session to connectivity issues will not be made up for at the end of a session.

 

I understand that appointments cancelled/rescheduled at least 24 hours prior to the start of the scheduled session time will not be charged. Appointments cancelled/rescheduled with less than 24 hours’ notice will be charged the full session fee.

 

Frequent cancelling may also result in a refusal of future appointments.
